113b-CODA Residency Category Exceptions Worksheet will summarize procedure codes categorized by CODA in multiple major and/or subcategories. 

Do not submit this worksheet in this format. Report 113a must also be run to ensure accurate categorical totals. This report contains PHI and should be handled in accordance with HIPAA policies and procedures.

You may customize this report by:

• Stage and stage start date
• Trainees
• Date range
• Role (Note: this filter will default to those contemplated by CODA Standards: Primary Surgeon, Primary Surgeon & Anesthesiologist, First Asst. to Attending OMS and First Asst. to Attending OMS & Anesthesiologist)
• Site (Note: some programs may want to report on an affiliated site. This filter choice can be made manually)

Only those trainees who entered codes which fall in to more than one major and/or subcategory will be reported on. Individual trainee report pages will be provided. 

Remember to update your categorical totals on the CODA Annual Survey after individual trainees have selected the appropriate categorical choice for each procedure shown on the 113b report:
