114-CODA Fellowship Worksheet will summarize procedure data entered to assist Fellowship Training Programs in completing Questions 22-27 of Part II -Oral and Maxillofacial Surgery Clinical Fellowships Curriculum Section.

Do not submit this worksheet in this format. This report contains PHI and should be handled in accordance with HIPAA policies and procedures.

• Stage and Stage start date
• Trainee
• Date range
• Role (Note: this filter will default to those contemplated by CODA Standards: Primary Surgeon, Primary Surgeon & Anesthesiologist, First Asst. to Attending OMS and First Asst. to Attending OMS & Anesthesiologist)
• Site (Note: some programs may want to report on an affiliated site. This filter choice can be made manually)

This report contemplates the number of times a procedure code was uniquely recorded. Counts are NOT based on unique patient MRN. Per CODA Standards, bilateral procedures are counted as separate procedures. Refer to the “Procedure Specific Tutorial” on how to log bilateral procedures to ensure accurate totals.

Some codes in Q22-27 are categorized in multiple major categories while other codes are categorized in a single category. Manually review each code in the worksheet, and determine which category is appropriate. If a code is only categorized in a single category, only one category will be indicated. Use “View Activities” and search by MRN to review each case in more detail. After you have selected the appropriate category for each procedure, total the appropriate number of procedures for each category accordingly.
